Participation Waiver & Media Release

Participation Waiver:

By signing below, I am agreeing that:

I understand that I am a participant or volunteer on projects/ programs, or events organized by Ecolibrium3 Urban Farm. I understand that activities may be hazardous to me including, but not limited to, spending time on a working farm in all weather conditions including sun, wind, rain, snow; and in direct contact with insects, mud, and other natural materials. All participants must dress appropriately: close-toed shoes, long pants, sun protection as needed (sunscreen, long shirt, hat, etc.), and for the weather/temperature (e.g. rain coat, jacket, snow boots).

Having read this waiver and knowing these facts and in consideration for the acceptance of my participation I, for myself and anyone entitled to act on my behalf, waive and release Ecolibrium3 from all claims and liabilities, of any kind whatsoever, arising from, whether directly or indirectly, my participation in Ecolibrium3’s projects, programs, events, or functions. This release of liability includes accident, injury, loss, or damages, as well as, to property which may result from the student’s participation.  I hereby release and agree to hold harmless Ecolibrium3, including its employees, from any claims arising out of my participation in the event(s).

Photo & Information Release:
I grant permission for Ecolibrium3 to use any photos, film, digital imaging, videos, verbal and written statements of the above stated participant or their likeness for promotional, web usage or other uses by the Ecolibrium3 project, event, function or otherwise.

If you select the no photo option, please let our staff know on the day of the event.
